The Quality Alliance 2011-2012 Year in Review is a celebration of the many accomplishments that were achieved over the last year. The Quality Alliance has moved from a concept into reality as a result of efforts dedicated to educating, collecting data, building metrics, reporting and introducing physicians to a new way to manage patients. This is an overview of the work completed and of the goals set forth as the program moves into the next phase of improving compliance results, introducing care coordination techniques and beyond.

2012 has been the year of fundamental accomplishments for the Quality Alliance (QA). Those activies have been related to network development, data collection, EMR implementation, metric adoption, compliance reporting, and implementation of an incentive plan. The aim has been creating the infrastructure for the program upon which we can build and move forward.

HCC Risk Scores and Your Medical Practice

Precisely identifying the diagnosis code could have a significant impact on the risk score. The data collection process starts in the physician practice with accurate and complete documentation for chronically ill patients at each encounter.  Consistent use of diagnosis codes at the highest level of specificity has a positive impact on the annual resource allocation.
 
CMS’s Implementation of the Hierarchical Condition Categories (HCC) model estimates healthcare cost associated with severity of patient condition.  There are 3,000 ICD-9 codes for 70 classifications designating chronic health conditions that are associated with higher treatment costs. Accurate risk stratification for resource allocation can be achieved through the practice of precise coding. Moreover, it is a significant component of our Medicare Advantage shared savings incentive plan.
 

Patient Centered Medical Home

The Quality Alliance has partnered with TransforMED for a pilot program that offers support and resources for QA practices interested in transitioning to this model of care.  Independent practices may be eligible to participate based upon QA membership and the completion of an assessment survey. The program has been proven successful for independent physician practices interested in maintaining a healthy work/life balance in the face of health care reform.  Additionally, the standardization employed by the PCMH model of care improves outcomes, increases patient satisfaction and improves efficiencies.
 

Buffalo Medical Group Joins Cleveland Clinic Community Physician Partnership's Quality Alliance

Friday, September 7, 2012, Cleveland: The Buffalo Medical Group has joined the Cleveland Clinic Community Physician Partnership's Quality Alliance, becoming the first physician's group outside of northeast Ohio to enroll in the program that helps independent physicians standardize care and improve quality.  Click here to read the full article.


The Employee Health Plan joins the Quality Alliance

Beginning January 1, 2013 the Cleveland Clinic Employee Health Plan (EHP) will grant "Tier 1" status exclusively to Quality Alliance members. The decision was approved by Cleveland Clinic Leadership an Community Physician Partnership (CPP) Board of Trustees.
